A person’s first visit with a neurologist is likely to include a detailed medical history and a neurologic examination It may include or be followed by diagnostic imaging, nerve testing, or other tests or procedures.

A neurological exam is used to evaluate a variety of functions, including motor and sensory skills, hearing and speech, vision, coordination, balance, and mental status — with the latter including mood and behavior. The neurologist may use a tuning fork, flashlight, reflex hammer, and a tool for examining the eyes during the exam.

During a neurological exam, the neurologist also evaluates the patient’s muscle strength, sensory ability, coordination, and memory. To get a better sense of what’s going on, neurologists may look at the way patients walk or button their shirts; assess their posture, balance, and hearing; and evaluate other areas of the body.

Neurologists often order imaging studies to see detailed images of the brain and evaluate brain function. Types of imaging used in neurology include:

Angiography: Uses x-rays and contrast dye to detect blockages or narrowing in veins and arteries. An angiogram may be used in conjunction with a clot retriever to treat certain strokes in which there is a possibility of unblocking the artery.
Magnetic resonance imaging (MRI): Uses magnets and high-frequency radio waves to create images of the brain
Positron emission tomography (PET): Produces 2- and 3-D images of brain activity that are used to find tumors and diseased tissue, show blood flow, and reveal changes to the brain following an injury
Single-photon emission computerized tomography (SPECT): Used to evaluate specific brain functions that may be responsible for seizures, degenerative spine disease, and stress fractures
Ultrasound imaging (ultrasonography): Uses high-frequency sound waves to create images of the inside of the body, allowing doctors to see changes in the soft tissues, including muscle and nerve.
X-rays of the chest and skull

Additional testing may include:

Biopsy (“tissue sample”): A procedure to remove a small piece of tissue so that it may be examined more closely. Muscle biopsies and nerve biopsies can be used to diagnose neuromuscular disorders.
Blood and urine samples
Cerebrospinal fluid analysis: After being removed in a procedure called a lumbar puncture (“spinal tap”), a small amount of cerebrospinal fluid — the liquid that surrounds the brain and spinal cord — is analyzed. The results can indicate certain metabolic diseases and neurological conditions, such as multiple sclerosis.
Electroencephalography (EEG): This test uses electrodes attached to the skull to examine the brain’s electrical activity. EEG is often used to diagnose epilepsy and other seizure disorders.
Electromyography (EMG): This test measures the electrical activity of muscles and is used to diagnose nerve and muscle disorders.
Electronystagmography (ENG): This group of tests is used to diagnose involuntary eye movement, dizziness, and balance disorders.
Evoked potentials (“evoked response”): This test measures the electrical signals to the brain that are generated by hearing, touch, or sight.
Genetic testing
Polysomnogram: This test measures brain and body activity during sleep and is performed over one or more nights at a sleep center. Test results are used to identify sleep disorders such as restless legs syndrome, periodic limb movement disorder, and insomnia, as well as sleep apnea and other breathing disorders.

Neurologists refer patients to neurosurgeons when testing reveals issues likely to be operable.

1. Angiography (অ্যাঞ্জিওগ্রাফি)

3. PET (Positron Emission Tomography)

2D ও 3D ছবির মাধ্যমে মস্তিষ্কের কার্যকলাপ, রক্তপ্রবাহ, টিউমার বা ক্ষতিগ্রস্ত অংশ প্রদর্শন করে।

4. SPECT (Single-Photon Emission Computed Tomography)

সিজার, মেরুদণ্ডের অবক্ষয়জনিত রোগ ও স্ট্রেস ফ্র্যাকচার নির্ণয়ে ব্যবহৃত হয়।

5. Ultrasound (অল্ট্রাসাউন্ড ইমেজিং)

উচ্চ-ফ্রিকোয়েন্সির সাউন্ড ওয়েভ ব্যবহার করে পেশী ও স্নায়ুর পরিবর্তন দেখা যায়।

6. X-ray (এক্স-রে)

বুক ও করোটির (skull) ছবি নেওয়া হয়।

🔹 Biopsy (বায়োপসি)

পেশী বা স্নায়ুর একটি ছোট অংশ সংগ্রহ করে মাইক্রোস্কোপে পরীক্ষা করা হয় — নিউরোমাসকুলার রোগ নির্ণয়ে সহায়ক।

🔹 Blood & Urine Tests (রক্ত ও মূত্র পরীক্ষা)

🔹 Cerebrospinal Fluid Analysis (CSF বিশ্লেষণ)

ল্যাম্বার পাংচার (spinal tap) প্রক্রিয়ায় মস্তিষ্ক ও মেরুদণ্ডের তরল (CSF) সংগ্রহ করা হয়।
এটি মাল্টিপল স্ক্লেরোসিস (MS) বা অন্যান্য স্নায়বিক রোগ শনাক্তে ব্যবহৃত।

🔹 EEG (Electroencephalography)

মাথায় ইলেকট্রোড লাগিয়ে মস্তিষ্কের বৈদ্যুতিক কার্যকলাপ পরীক্ষা করা হয়।
এটি এপিলেপসি (মৃগী) ও অন্যান্য সিজার ডিসঅর্ডার শনাক্তে ব্যবহৃত।

🔹 EMG (Electromyography)

পেশীর বৈদ্যুতিক ক্রিয়াকলাপ পরীক্ষা করে স্নায়ু ও পেশীর সমস্যা নির্ণয় করে।

🔹 ENG (Electronystagmography)

চোখের অজান্তে নড়াচড়া, মাথা ঘোরা ও ভারসাম্যহীনতা মূল্যায়নের জন্য ব্যবহৃত হয়।

🔹 Evoked Potentials

শব্দ, স্পর্শ বা দৃষ্টির মাধ্যমে সৃষ্ট বৈদ্যুতিক সিগন্যাল মাপা হয়।

🔹 Genetic Testing (জেনেটিক পরীক্ষা)

বংশগত স্নায়বিক রোগ শনাক্তে ব্যবহৃত।

🔹 Polysomnogram (Sleep Study)

ঘুমের সময় মস্তিষ্ক ও শরীরের কার্যকলাপ পর্যবেক্ষণ করা হয়।
এটি নিদ্রাহীনতা, স্লিপ অ্যাপনিয়া, রেস্টলেস লেগ সিনড্রোম ইত্যাদি নির্ণয়ে সহায়ক।

🏥 চিকিৎসার পরবর্তী ধাপ

যদি পরীক্ষার ফলাফল দেখায় যে কোনো স্নায়বিক সমস্যা অস্ত্রোপচারে (operable) সমাধানযোগ্য, তাহলে নিউরোলজিস্ট রোগীকে নিউরোসার্জনের কাছে রেফার করেন।
